Your new role

To work alongside residents aged 18+ to facilitate their safe and timely transfer from hospital in line with Welsh Government hospital discharge service requirements and utilising discharge to recover and assess guidance. Facilitating timely and safe discharges from an acute setting, liaising with professionals, the individual, their carer's, family members and all those involved in determining a person's correct discharge pathway. To act as the main contact with inpatient acute care in the out of county district general and community hospitals to ensure safe and effective transfer of patients back into the community. You will be doing this by identifying obstacles and working alongside system partners and the Hospital Social Work Team to unblock these obstacles so that residents can return home from hospital without unnecessary delays.

What you'll need to succeed

  • Degree in Social Work, DipSW or equivalent
  • Registration as Social Worker with Social Care Wales / CQC
  • Post qualifying experience of working with Older People / younger adults and their carers
  • Experience of undertaking assessments of need and identifying and managing risks.
  • Experience of complex casework
  • Experience of care management across a range of health and social care settings


What you'll get in return

  • £34,373 to £36, 370 per annum £17.8166 to 18.8516 per hour
  • 37 hours, Permanent
